Our Mission
St. Vincent's mission is to make a positive difference in the lives and health status of individuals and communities. Central to our mission is service to those persons who are poor. The health services we provide will be spiritually-centered, accessible and affordable.
The Diabetes Education Center enhances this mission by providing quality services to patients with diabetes and focusing on health promotion.
Patient Education
The Diabetes Education Center is a self-management education program. This diabetes self-management education program has been awarded recognition by the American Diabetes Association in accordance with the National Standards for Diabetes Self-Management Education Programs.
Our instructors include staff specializing in the care of persons with diabetes including nursing and nutritional professionals who are Certified Diabetes Educators.
The Diabetes Education Center at St. Vincent's Hospital is dedicated to helping persons with diabetes lead a healthy lifestyle. Through classes and/or individual instruction persons with diabetes are taught self-management skills for controlling diabetes. Our program takes a multidisciplinary approach to diabetes management and benefits patients by involving a variety of healthcare disciplines.
The Diabetes Education Program includes:
- An individual assessment to tailor diabetes education for each person with diabetes.
- Instruction for insulin administration and home blood glucose management.
- Individual nutritional assessment for each patient by a registered dietitian and an individualized meal plan based on each patient's nutritional needs.
- a comprehensive diabetes education program with content as recommended by the American Diabetes Association.
- A specialized exercise program on the St. Vincent's campus is offered to all participants.
- A free foot screening and assessment.
- Encouragement of participants to set realistic goals for specific behavioral change related to self-management of diabetes.
- Follow-up education and support meetings are offered for all participants to reinforce diabetes education.
Physician referral is required for participants in the Diabetes Education Center. Physicians will be kept informed of their patients' progress in the Diabetes Education Center, and will be sent a summary of the education sessions their patients attend.
