Dear Clinical Instructor,

Thank you for choosing St. Vincent 's Health System (STVHS) for your student's clinical experience. We welcome you to our facilities.

In order to become familiar with some of the STVHS policies, we have compiled an online manual to deliver information you and your students need to know before beginning your clinical experience. Reviewing the information in the manual is essential and must be completed prior to the first day of your clinical rotation . Validation of the review of the materials is done by completing the forms listed below. You will also be instructed to contact the clinical liaison for the facility where you will be holding clinical rotations for further instructions and additional materials to complete prior to your first clinical date.

If the link doesn't work, copy and paste into your web browser. You will be able to review your manual online or print to your local printer. Once you have reviewed the material, you should complete the following forms:

• Codes/Test
• Mini Orientation Checklist
• Orientation Checklist/Acknowledgement Form
• Statement of Confidentiality
• Manual Receipt Verification.

In addition, you will need to complete hospital and department specific competency assessments.

Your students should review the online manual and complete the following forms:

• Manual Receipt Verification
• Orientation checklist/Acknowledgement form
• Statement of Confidentiality
• Test

These completed and signed forms (4) must be completed by each student prior to the first clinical rotation. You should have all four completed forms for each student when you meet with your clinical liaison prior to your first rotation.

Please note: STVHS believes that the individual is responsible for reviewing content. Your signature on the acknowledgement form indicates you are responsible for the material contained within the manual. A signed confidentiality statement is required before clinical rotations can begin.
