Thank you for choosing St. Vincent 's Health System (STVHS) for your clinical rotation experience. We welcome you to our facilities.

In order to become familiar with some of the STVHS policies, we have compiled an online manual to deliver information you need to know before beginning your clinical experience. Reviewing the information in the manual is essential and must be completed prior to the first day of your clinical rotation . Validation of the review of the materials is done by completing the forms listed below. Your instructor will give you additional instructions for turning in the completed paperwork before your first day of clinical rotation.

If the link doesn't work, copy and paste into your web browser. You will be able to review the manual online or print to your local printer. Once you have reviewed the material, you should complete the following forms found on the same page with the manual:

• Manual Receipt Verification
• Orientation checklist/Acknowledgement form
• Statement of Confidentiality
• Test

Once completed, bring the completed forms (4) to your instructor. Please indicate the facility where you will be attending clinicals on each form as requested.

Please note: STVHS believes that the individual is responsible for reviewing content. Your signature on the acknowledgement form indicates you are responsible for the material contained within the manual. A signed confidentiality statement is required before clinical rotations can begin.
